Indiana State High School Atheltic Association requires every athelete to obtain a physical from a medical doctor clearing him or her for performing any extra curricular athletic activities. It is extremely important that these physcials get turned in by August 4th 2008.
No athlete will be allowed to partake in any Fall Football activities without this physical.. No exceptions!!
Please click on the form below and print it out. Take it to the doctor with you. Have it signed by your doctor and your parents and turn it in by Aug. 4th 2008.
